Who do I contact if I have a billing question?

What do I need to bring to my office visit?

Before initial visit, new patient forms must be completed. You can find them here. You must bring your Driver’s License and one other form of Photo I.D. and a current copy of your Insurance Card.​

​How do I get an appointment?

​Please call our Front Office during normal business hours to schedule your appointment. We will not accept any appointment requests that are received after normal business hours.

​What if I need to cancel an appointment?

​Please contact our Front Office 24 hours prior to your scheduled appointment. If you fail to do so, cancellation fees may be applicable.

I recently had lab work done at the office. How can I find out the results?

​Normally, you will be notified of your results within one week. However, if you have not heard from someone in our office within two weeks please call the office to inquire. Did you know that some of your lab results may be sent to you via our Patient Portal (secured site governed by HIPAA Regulations) If you have not yet received your confidential log-in information please call our Front Office during normal business hours.
